Displayed below are links to PDF forms designed to assist you. When submitting a form please make sure that all the required fields are completed and that the signed copy is delivered to the Office of the Registrar.

  1. Administrative Clearance Form
    Use this form to receive administrative clearance approvals for graduation. This form must be submitted two weeks before the end of the semester you intend to graduate in.
     
  2. Application for Graduation
    Use this form to apply for graduation in your final year within the time frame specified by the Registrar's Office.

  3. Classroom Reservation Request Form
    Use this form to request a classroom for an academic event.

  4. Consent to Disclosure of Academic Information
    Use this form to release academic information.

  5. Credit Hours Increase Request
    Use this form to request an increase of your maximum credit hours.

  6. Degree Program, Major and/or Minor Declaration Request
    Use this form to declare your Major and/or Minor.

  7. Degree Program Major/Minor Update Request
    Use this form to update your Major and/or Minor.

  8. Grade Appeal Form
    Use this form to request a change in grade.

  9. Incident Report Form
    Use this form to report incidents on campus, accidents, and concerns about behavior.

  10. Independent Study Request
    Use this form to initiate an Independent Study course.

  11. Internship Request
    Use this form to initiate an Internship.

  12. Individual Course Withdrawal
    Use this form to withdraw from your last class.

  13. Leave of Absence
    Use this form to request a leave of absence from AUK.

  14. Pass/Fail Request
    Use this form to take a course pass/fail.

  15. Permission to Audit
    Use this form to audit an AUK course.
     
  16. Request to Override Course Capacity
    To request a course capacity override. Be sure to follow all instructions.
     
  17. Request to Remove Academic Enrollment Restriction
    To request a Prerequisite, Permission of Instructor or Class Standing override. Be sure to follow all instructions.

  18. Student Name Correction
    Use this form to amend your name for your academic records.

  19. Single Course Drop/Add
    Use this form to Drop a course you registered for or Add a course to your schedule.
     
  20. Study Away Request
    Use this form to request permission and credit for study away from AUK.

  21. Transcript Request Form
    Transcripts can now be requested though Self-Service. Save a trip and order on-line. If you want to print this form you may do so, but it requires an extra trip to the Registrar's Office - one trip to turn it in and another the next day to pick it up.

  22. Withdrawal from AUK
    Use this form to request permission to withdraw from AUK.
       
  23. Withdrawal from a Semester
    Use this form to request permission to withdraw from a Semester.
